Domestic Partnerships

Applicants for domestic partnership must submit a notarized affidavit of domestic partnership and respond to questions on Worksheet for completion of Certificate of Domestic Partnership. If applicants meet the criteria established in the worksheet and have completed the notarized affidavit, a Certificate of Domestic Partnership will be issued. The fee for the Certificate of Domestic Partnership is $28.00 and there is no waiting period. Domestic Partnership applications are by appointment only.
